Is there a website somewhere with a simple step by step list of what I need for a simple conservatory? We're in a maisonette so need permission, which the man from planning said should be a straightforward thing, he came round because my neighbour phoned him up to complain I was digging foundations, actually I was just cleaning the area to see what the ground was like.
I know I need to fill in the form, but what do I need to send in with it? I'm in Hampshire if thats important.

You would need a ground floor plan as existing and proposed, plus elevations and maybe a cross-section.
Also a 1:1250 scale location plan showing the site and adjoining buildings/streets.

I've got a floor plan in the deeds, can I use that and draw on the extra bit?

You don't need a Design and Access statement.

You can use the plan from the house papers for a location plan.

PS - just re-read; a floor plan won't do for a location plan, and may not do for the main plan, though you could trace over it.

On the bottom of the form it says 'Are you supplying additional information on submitted plan(s)/drawing(s)/design and access statement?'
So I can just send a location plan and a site plan, which are probably both in the deeds, but dont cost much to do online anyway.

Yes - have a look online at the stuff most people submit. Some of it's terrible, so credit to the LPA validation teams (not a phrase that you'll often see me write) for accepting it!
