door lining

Hi Fellas,
I plan to change a couple of my downstairs doors and lining as the previous occupier had a strong liking for staining and varnishing everything!
Anyway, i was wondering if anybody could give me a guide on how to replace my door lining. I have read a couple of various methods like making it first then offering up to the doorway or putting the head up and the hinge side of the jamb then offering the door up and then fitting the other jamb.
So, i guess iwould like to know
1. Which is the best method
2. Is it advisable to purchase a "door lining set" from one of the sheds
3. Which type of fixings are best to use

frame packers /folding wedges held by nails then screws and plugs if masonery or just screws if wood
you will probably have to buy a new door as the old frame is likley to be out off square

you may have to remove some masonery/brick/wood to allow the frame to go in square and level!!
