Do all extensions need planning permission, I seem to remember if its a certain percentage of the size of the existing house i=then you didn't need permission but I could be wrong?

The Permitted Development (PD) allowances used to be calculated on volume of proposals. However, this changed on 1st October 2008. The Planning Portal is a good guide to give you an idea as to what may or may not require permission. As it's only used as a guide, only your local planning department can give you the definitive answer as there may be other hidden restrictions.

If in doubt, ask :)
 
The correct answer is that yes all extensions need permission, but some are "pre-approved" by way of permitted development rights.

If your extension does not conform to permitted development, or if permitted development rights do not apply to the property, or if they have been removed by the council, then you need to apply for permission

It will still need Building Regs. approval though; don’t confuse this with Planning permission, they are very different. ;)
