Vibration on Citroen C15 van

I'm getting a weird side-to-side vibration in a Citroen Champ C15 diesel, which comes in at about 40mph and goes away at about 50mph. I can't feel it through the steering wheel (although I've checked all the tyres for balance and lumps anyway). While it's happening, the gearstick is chattering from side to side and the whole van seems to shake side to side as well. Anyone encountered this, or have any ideas about what it might be?

See if the driveshafts have anti vibration mounts on them (like a rubber donut) as these can fall off / move causing problems.

I'd have another look at the tyres though, and also do a diagonal swap
 
Sounds like a worn driveshaft. With the front jacked up try to spin the wheels (by hand) Often if the driveshaft is shot it will lock the wheel up with a heavy bang. The first time I saw it I was convinced there must be debris locking the gearbox up internally, it's such an aggressive lock up.
