If I take my kitchen door off its hinges, do I then have an open-plan kitchen extending into the hallway?

No, you have a door frame in between with hinges still attached. Two separate rooms.
What if you also removed the door frame and made good?

What if a small shower room connected to the hallway had the door removed. Would you have an open plan kitchenbathroomette?
