shower/exposed or built-in

I've just purchased a shower from screwfix (a combi shower).
After looking at the instruction manual i'm still no further forward. The shower can be either concealed or exposed. The exposed looks simple enough, the hot and cold 15mm pipes are connected to the inlets at right angles to the wall. The flexi pipe is then connected to the mixed water outlet and this leads to the shower head itself.
My problem is if it is concealed how do i get from the mixed water outlet to the shower head? Do i have to use a bit of copper pipe recessed behind the wall to the shower head at the top?
Sorry if this is a bit confusing but the instructions are poor

Usually yes, with a different shower head bracket thing. SOmetimes concealed mixers still have the hose outlet in front of the tiles though.
 
The shower did not come with any type of fitting to connect the mixer valve to the shower head(concealed). They have supplied a male wall outlet but 15mm does not fit on to this it may be half inch. If anyone could link me to a website which shows you how to plumb up concealed fittings it would be much appreciated,
