Has anyone heard of this before? Apparantly it's to do with the old W Plan, 3 port divertver valve.

Is this when the valve stickins in the htg port open position and so hot water then when on heats up the rad circuit?

you mean reverse circulation.
can happen on w, s, y plans.

when you have 2 radiator circuits joining the primary return.
both radiator circuits need to join together then join your primary return
need to follow the 3 Tee rule.

A 3port valve passing to heating because of siezing is just that and nothing to do with reverse circulation, which is a piping design fault.Often happens when extra rads are added to a system and the return is connected in the wrong place.
