How to smooth very bad skirting and door frames

Hello, I am about to start glossing my starcase and door frames, but i have noticed that they are in quite bad condition (bubbling effect on frames).

What is the best solution to get as smooth a finish before i start painting. I have tried to sand them down but this has not made any difference. I am about to try a heat gun to remove the old paint back to the wood.

Difficult one to answer. Maybe best getting in an Paint Removal Specialist.
 
It is often easier to pull off the old joinery and affix new.

Not when radiator pipes are concerned

Oh, and when the fixings are nails in wooden plugs in the wall.

I was in the same situation as you.
used heat gun stripped back to bare wood. long and tiring process, but you'll get there in the end
