central heating problem

help please i have a system that has been working fine but suddenly i find that the system is constantly expanding and i have a constant flow of hot water flowing into the header tank and then back into filling the system. (it is not overflowing from the tank).... preeceding this there was aloud rumbeling from the middle floor of the house much like water hammer. ... the boiler is reporting that the difference between the flow and return temperatures is too great and is shutting off prematurely all the radiators are luke warm....i have checked the pump and it is working fine .... any help appreciated
