DHW flow switch is very poor frequently breaks down but you could as easily need a ne diaphragm. Sounds to me like you have done all you need to do to the plate heat exchanger as long as what you did was to the black holes not the clean ones.
The white temp sensors (thermsiters) are bullet proof but if you have other colours they might be implicated. Can be either one or both the boiler uses the primary one as an overheat sensor during hot water use. It seldome would be these sensors however.
make absolutely sure there is no leaching of cold water into the hot through the shower or a mixer tap. To test for this shut the cold water supply to the boiler and open a hot tap if water comes out you have leaching. To fix it go round the mixer taps and showers to hear the water flowing across from cold to hot pipework which is how come it comes out the hot tap even though the feed to the boiler is cut. If this is happening the suspect appliance has to be changed or put single check valves on both pipes leading to it. This behaviour plays havock with combi hot water performance.
