Warm Deck Flat Roof

Evening all,

Just had an extension built with a new flat roof. Effectively it's been done as a warm deck, with OSB/Ply then Reticel insulation, then another layer mechanically fixed, then the felt.

Issues-

1. No vapour control layer
2. Is this insulation correct to have a mechanical affixed further layer over?

Would really appreciate some advice, as I am getting conflicting views over the insulation being incorrect.

Pretty normal to put another layer of timber over the insulation. Ideally that should fixed with special screws that minimise thermal bridging but it's not the end of the world if normal screws were used. I think most people would add a separate vapour layer but some consider the foil faced boards to not need it. I'd have gone for epdm instead of felt. Not much more cost for a much better surface but apart from that I wouldn't be too worried.
 
Thanks for the reply. They've not put ply over the insulation, just screwed that black layer onto the insulation.

This has all come to light as the final felt was terrible and so I've called in roofers to give advice
