internal manhole cover

Floor that I intend to tile over has a manhole cover flush with the screed.
I am not sure of the best way to tile it.
1. Tile directly onto the existing cover and cut tiles to suit. And leave lose the tile sections over the access levers which lever out. although I am not sure how I will be able to lift the handles to the correct working position.
2. Use a Howe Green access cover over the manhole cover. The size of the Howe Green cover is pencilled onto the screed as in the picture.
The screed butts against a ply covered floor so I will be using a decoupling membrane onto the screed.
