That looks like a programmer/timer that you’re trying to replace. There appears to be an additional live and neutral, which could be the boiler feed. Do you have an existing thermostat?
Essentially it’ll be all Lives to L, all Neutral’s to N, and 1 to 1 and 3 to 2 on smart stat respectively.

Yes there are 4 wires from boiler (2 wires to L and N) plus 2 cables connect to 3 and 5
