Little confused on this. In order to extend to full height, which end goes into what as no way you connect to one another appears to be correct?

If you've got 6 bolts, they are not joined.

They are a universal UK/Australian fitting.
 
Fix the first one in, then stick the next one on top so the top slots line up enough to get the bolt through.

Probably a stupid question, but here we go... with regards to wall ties, what if your inner and outer skin are built off different levels of ground, therefore the mortar lines do not match, do you just bend the wall ties up or down to meet the mortar joint?

Always bend downwards(if level is not possible) towards outer to prevent water tracking to internal wall,sometimes its better to use longer ties if bending down,i presume your using imperial size bricks and metric block, and if not just use cut block or brick to get to right level.

How would that even occur? Both leafs of a cavity wall are off the same foundation.

Kevin, I fitted exact one today, if you notice both strips are the same, and each one has one end closer to a slot than the other end.

Take the ends with the slot closest to the end and slot the two together. One of your five bolts goes in that “joined up” and overlapped slot.

then use two bolts for the bottom strip, two bolts for the top strip.

The internal inner skin is being built off self levelling compound, the outer is being built off brickwork. Probably using heavy blocks for the outer and thermalite for the inner.
