Lost my boiler service book! Intergas

I had a Intergas combi installed in November. However I have misplaced book!!

I have 12 year wants ceritifacate. Is there anyway around it? Can I get a replacement book? As it a first service is not due yet? I am just worried about commissioning checklist which I presume is in book?
