Pressure valve

My pressure release valve seem to be operating to often, releasing hot water from the boiler , Why?

PRVs are supposed to release pressure at 3 bar. What system pressures do you have when cold and when the PRV operates?
 
A. the expansion vessel is duff.

B. the expansion vessel is under charged.

C. the expasion vessel is too small for the system.

D. the system pressure is too high to start with.

E. the secondary HX has a pin hole.

F. the pressure relief valve is duff.
