garden retaining wall

can anyone help. i built a retaining wall 5foot above ground level by 6meters long and the founds were were 400mm down with a base 500mm wide by 150mm thick. It was a single concrete block wall. At ground level i backfilled with very small gravel [ large bag from keyline] then i reused the soil i dug out to backfill.After heavy rain the wall was pushed down. Where have i gone wrong.

I'd hazzard that you guessed rather than paying for it to be designed. 150 thick block is nowhere near thick enough and the foundation sounds undersized too. Should've been more like this:
