Possible causes..
Leak in system
Faulty PRV
Or the more likely cause of a faulty Expansion vessel.
Drain your system until pressure is 0bar and then check the pressure of your expansion vessel, you can use a tyre pressure gauge for this as it's a schrader valve. The recommended pressure for your expansion vessel should be written on the vessel itself or within your manufacturers literature.
Also press the valve with your finger and see if any water comes out as this means the diaphram inside has split.
If there is no water but the pressure in the vessel is low, use a bike pump to pump it back up to recommended pressure, top your system up and see how it goes.
