Floor fitting ideas?

I want to fit some vinyl floor over my concrete floor in the kitchen.
Ive got most of the holes out using a mortar mix and ive cut some 6mm ply to underlay the floor.
What should i use to stick the ply down? Some no more nails? Any one suggest anything better?
Is it advisable to install some sort or plastic or damp membrane below the ply or should it be OK as it is? The room can get fairly damp and cold in winter as it is north facing.

You would be better using a smoothing compound on the floor.
Then a liquid dpm onto of it.

What floor covering you looking to use ontop?
