Why do you want to lay ply down.
If your fitting Parquet just fit this straight to concrete
If you really want to fit the ply you can use evostick gripfill and plug and screw the ply down "2x10 screws + brown plugs.
Put plenty of screws round the egdges as well as the middle, approx 20...
Only 18mm T&G hardwood floors can usually be used nailed straight on to the joist. But check the manufacturers specification, max centers etc...
Why take the old floor up?
If it is ok condition leave it down and thicken up your floors timber is a great insulation
If any timber is rotten you should replace it.
For removing the old paint just scrape then sand the loose off.

Sounds like the sub floor was not within the tolerance of the floor laid.
Or something has been left under the floor . ie bits of wood etc...
You will probably have to rip the floor up, level the sub floor and start again :shock:
