I need to raise the floor in my garage up to meet the hallway. the difference is 6 inches. what is the best way to do this? Do I need to put a damp membrane down first? I have searched Google but can't find any real guides.

You need a dpm if you are unable to confirm one is under the slab already though for the small cost you should fit one anyone. Then use 140mm polystyrene insulation and floating 22mm T&G plywood boards on top. This will also get you to the required U Value. Stick a solid joist under door thresholds.
