Hope that was an actual manhole and not just an access into a floor void. Liked the very non-level floor as well.......
 
1. He needs to move.
2. It's a good point to drain the heating system.
3. I like the silicone around the cover, that will stop the leak. :eek:

Nothing wrong with a 'Build over' provided its done properly. Internal chambers are best avoided, but if they simply have to be there, then double sealed, screwed down covers are a Building regs requirement, to prevent exactly the type of thing that's happened here.
