Can somebody explain in simple terms what I need to do for my door?

I wonder how that effects thermal efficiency, since it increases thermal mass.
The floor is no different to the inner skin of masonry in terms of thermal mass etc. We would rather get the stuff in and under the slab so that we have only a 75mm step down to contend with throughout the build.

yes, that is better in terms of thermal mass as the insulation keeps the heat in the concrete slab.
