Laying a new drain under a concrete slab

I need to lay a new drain from my bathroom under a new concrete slab to the rear of the house where i am installing a new drain. I can not find any details anywhere on how to do this. Plenty on installing new drains outside.

I have about 600mm from floor surface to the bottom of the pipe so should have plenty of room for approx 100mm screed, insulation, slab, blinding an hardcore.

Does the drain have to go under the hardocre? or in it?
How does the drain penetrate teh DPC when coming up vertically at the end of the run for W.C.
How do i connect other sanitary ware mid way along eg bath, basin, shower.
Anyone know of any drawings etc?
