little rcd tripping puzzle

Little teaser for the non qualified here.
Called to a job, dustproof fleurescent fitting wired to a plugtop plugged in to rcd protected circuit.
Worked for two years till someone tripped up and pulled flex out fitting.
Maintenance refitted flex but now trips rcd circuit when plugged in, but works ok on a non rcd circuit.
Plug was fitted ok and site said they had connected fitting
BROWN to L
BLUE to N
What was wrong

Where did they connect the earth from the flex to?

By the wording of your response I'm guessing the third terminal was not intended for the cpc

Did it have 4 terminals?
