Can anyone advise on how to best position this? I am fitting a Charnwood 4 http://www.charnwood.com/range/stove/cfour.aspx and also need to know what height above it's top surface I need the opening (lintel). Not sure how the plate fits in relation to the lintel. Thanks.

Thanks, the instructions only suggest 300mm but gives no details about this plate as it is not really part of the spec. I am an experienced DIYer and will be applying to th LA myself. I see now plenty of demos on U-Tube and have a good idea now what to do.

There is a lot of info in document J which you can download for free
If you are fitting a liner then it's a closure plate that you need to fit
If possible try to get the joint between the flue pipe and liner adaptor slightly below the closure plate. The reason for this is is to inspect the push fit joint after sweeping.
