Suprima 30 fan whining noise

When my Suprima 30 boiler starts up the fan is whining quite badly, once "warmed up" the noise abates a little and at time goes away. On odd occasions the fan is normal.

The boiler is regularly serviced (in the autumn), is it likely the fan could fail and be a safety issue?

Would a replacement fan be best bet and if so, what would rough cost be to replace?

The fan could fail, but if it does so it won't be a safety issue - the boiler will just refuse to light. Fans for these are about £200 plus the cost of fitting. Budget £350 total and you'll probably be about right
