This may sound stupid but I am in a new house with many inset spots in the ceiling. My first bulb has just gone but I can't work out how to remove the bulb. There is a small metal spring that appears to hold the spot in place but when this has been removed does the bulb twist to come out or do I have to ease it out with something sharp?
help please!!!

Do you know if the lights are mains voltage or low voltage ?

if you mean by spring a circle of metal around the perimiter yes, you remove this and the lamp usualy falls out (still connected to its cable)

Turn the lighting circuit off, then remove the circlip, and the lampholder will drop down, the voltage will be on the lamp. Then low voltage lamps pull out and mains voltage need to twist and release, generally.

Beware of very hot lamps !
