Having brought all the stuff i needed as per above details i have found that the floor is out by around 10-15mm further into the room. Most of it starts at 75mm when i come off the original floor but is upto around 85-90mm over the far side of the room.

Right having spent the afternoon trying to dig it out (have only got a little hole at the front) i have reached the soil/sand underneath. There is a metal wire going through the concrete at approx 70mm down (see image below). How can i cut that without special tools? I also have the wall...
The concrete floor that you can see has got to come up about 70mm but will be done with a framework and ply on top so hopefully pipes will run underneath somehow.
If not then i will need to channel out around 2 1/2 metres each way for shower/sink and bath.
What sort of drop will i need?
With foresight i would have visisted the place 2 years earlier before the person that built the extention died. That way i would have known what he had in mind :?

I think the soil manifold is the way to go.

Ok thats fine.
Going back to the sleeper joists around the perimeter i understand them but do the other 400mm sleeper joists have to sit on top of the these or just inside those, as if they have to sit on top i don't think there will be enough depth for it all.
