Opening up the fireplace after removing the cast iron insert and find a rounded lump of extremely hard concrete in each of the back corners.
Suspect the insert wasn't original (house is 1906) due to the mortar and rubble that was behind it which came out very easily with an SDS.
Did these lumps in the corner serve any purpose and can they be removed ?
