isolation ball valve questions

hi I have a leaking isolation ball valve on a feed to a radiator.
it has been in for a year with no problems but when I switched it off to change the radiator it leaked where the screw bit is.

I am going to replace it but I wondered if the problem was maybe I had on the wrong way wrt water flow/pressure. What problems occur if you fit an isolating ball valve the wring way round.

Is there a way to work out which is the feed and which is the return pipe - nb when i disconnected the radiator the connecting pipe drained out much darker water on one side than the other side - Maybe dark side return, clear side flow?

any advice /views appreciated gratefully
