After fixing this I finally got the thing to budge. It seemed like pushing the light bulb out of it's fitting and up (so it's kinda just hanging in the roof) and then sticking you're fingers around and poking at the metal ring helped. Still needed a lot of force to get the thing to twist though.

I've replaced three GU10 fittings like the one below without issue. It's been as simple as grabbing one or two of the lugs and twisting anticlockwise. The inside white ring then twists about a centimetre and then falls out from the middle ring. The light blub with then fall down slightly, and...
