Opening up the fireplace after removing the cast iron insert and find a rounded lump of extremely hard concrete in each of the back corners.
Suspect the insert wasn't original (house is 1906) due to the mortar and rubble that was behind it which came out very easily with an SDS.

It's a SIAMP Trueflow cistern and a flavour of SIAMP fill valves, most likely a Compact 99B. Changing the whole valve has to be the last option due to access to the bottom of the cistern. Do these valves dis-assemble ?
