Could be a few things, all easy enough to fix/replace. Could be washer on bottom of flush valve not seating correctly or prongs on bottom of push button too long and holding valve open. Most likely though will be that vvank armitage float valve, just replace that with a fluidmaster and alter...
Are you sure you need a 3/4" extension? usually the 1/2" extension will go into the rad, then the old tail into the female end of the extension so that the 3/4 nut then meets back on the old valve. Like Petit Pablo said, a picture speaks a thousand words
Water Boards have to supply a minimum 9 l/min at your boundary, i'd be checking your neighbours flow rate, if thats greater than yours then you know the problem lies after where it splits off to feed your property.
Who's the manufacterer of the stoptap? Plasson do adapters for their poly stoptaps, but doubt they'd fit other manufacterers.

What are you wanting to mount one to the wall for? Have a look at a surestop valve, fits after your stoptap and then can wall mount it like a socket or hide it away in a cupboard
They run in line with the hearth and this seems to be the bounciest part where the hearth was resting on this joist although it does it slightly in other places
