    After all that....what an anti-climax. I found that problem on many occasions and always when the cold feed dipped before connecting into the flow. I always tee'd cold feed into the open-vent just before the connection to the flow and never experienced any blockage problems.

    Are the rads getting hot, or just slightly warm? If slightly warm then it could simply be the boilers pump over-run dumping hot water from the heat exchanger into the system when hot water demand ceases.

    The make and model of the boiler would be helpful. It sounds like either the boiler stat or pump over-run stat may be faulty. A faulty boiler stat would cause hot water to be dumped into the expansion tank, this could also cause the open vent pipe to become detached from it's clips and drop...
