We have very old (and crumbly) insulation in our loft. Is it OK to just put the new insulation on top of this or should we completely remove the old insulation first?

I believe there was 1 T junction after it came into the kitchen. 1 routed the cold water upstairs and the other to the boiler through the kitchen where it came up via an elbow into pipeing for the boiler. The pooling doesn't occur near either of these but thats not to say they are not the source...

Just out of interest, anyone know what could cause these pinhole leaks in the green pushfit scalemaster? I would have thought you need quite a bit of force to do this (we did upgrade to a 50mm blue pipe for our cold water supply)
any more recommendations on a good quality water softener?

Just an update. My fitter returned and made an adjustemnt to a valve which I beleives connects the flow and return and this seems to resolve the issue with rads now heating up as they use to.
