Most problems fixed:
- Shower now vertical.
- Shower outlet in the right place.
- Floor fully trussed and rock solid.
- Wall tiling done and looking excellent

I pointed out the shower valve and outlet and he seemed very happy to fix these.
I only realised that he wasn't going to strengthen the floor when I saw that he's already screwed it down - and installed the bath on it...
So I guess that he probably is going to have to remove the bath...

You haven't quite got it right. The hot temperature INLET is 65C and that's fine. The thermal element only shuts the water off if the MIXED water temperature exceeds 45C and this would only happen if the cold water ceased. (The thermal element is a wax-filled capsule that sits in the mixer...
Well, to their credit, Trevi did eventually send an engineer to have a look. He spent about 4 hours with me, during which time I completely re-plumbed the cold in 15mm (thank god B&Q are close and PolyPlumb is quick to use!), removed both single check valves on hot and cold and tried several...
I had a discussion at work with one of the egg-heads of our mechanical engineering department and his conclusion was that it would make bugger-all difference to run a 15mm feed to the unit.

Already tried removing the outlet hose and blasting water through (and even re-routing the outlet pipe so no airlock was possible... but no joy.
Also tried removing the mixer cartridge and reverse flushing it as suggested by...

They reckon that the problem is that the cold is plumbed with 22mm and not 15mm.
Not sure why this would affect the shower. 15mm would affect flow pressure and flow rate, but this would be dependent on the length of 15mm pipe and number of bends.
