The balanced cold take-off on the ICG is before the NRV, so the hot usage should not make a difference to the pressure on the cold side of the ICG.

Do you have a toilet valve that shuts off quickly?

Litres per minute from the main is the most important point here.

It doesn't really matter. As long as it's secure because the weight of the arm and head will make it slope if not. I usually flush it with the backer board. If the thread on the arm is too long, you can always cut it down and if too short, you can use radiator extensions to bring it forward.
