These Boilermate’s didn’t have a mixing/blending valve on them like the earlier ones. Do you get scale where you live? Possibly scaled domestic plate heat exchanger or a mixer tap somewhere in property faulty and passing cold water to all taps.

Not sure on that model but there’s usually either screws on top or bottom to loosen and programmer will come off of a backplate. If no screws there will maybe be a couple of slots to get a small flat screwdriver or similar into to prise off. There must be something or how would installer...

With that boiler, if happening as frequent as that, it may not be nest related at all. Get a registered gas engineer in to look at boiler.

Could just be your boiler thermostat is set a bit lower than it was previously and is coming up to temp before nest set temp is reached. Is the nest in same place as old one was? Maybe the old stat wasn’t readings temperatures bang on ie degree or two out, maybe the nest isn’t calibrated...

if the ch 2port isn’t motoring across then valve itself may be passing. The whole valve body can be replaced or you can get service kits for changing internals only. They can wear down over time or get muck preventing them closing properly.

Unless I'm getting something wrong here with the figures, the gas rate for the figures you provided for one min of hot water use is very high. With using 0.188 as my difference between two readings over 2 mins i get this as a gas rate
