I think it would look gash and different materials behave differently in changing temperatures etc. Not sure why you wouldn’t opt for a nice composite door or new UPVC Door panel which will be far easier. Looks like you would need to reduce the door height to have a wooden frame below the upper...

Are you looking to cover the opening all together or plasterboard the rafters so it will be more detailed?

Not sure it’s a goer but my initial thought is to use concrete fence repair Spurs. You can mount them in front of the concrete divider. Check out YouTube for inspiration

I’m not sure I would use polyfilla on concrete. It isn’t clear how bad the damage is but Can you not remove the filler, mix up a small amount of concrete and skim the top?

I would and would likely opt for a slab rather than a roll. Would be easier to slide into the cavity.

That’s good advice. As mentioned give some thought to the furniture location and then access to the door. If I was designing ours, the middle opener would have been preferable.
