    Sounds to me like the previous paint was not bonded properly to the timber, I would advise to prime the bare wood prior to any painting. After priming you could fill the areas to blend them back in.

    If it is possible then loosen up the screws that are holding the cistern onto the wall and put some spacers between the wall and cistern, I feel it is being caused by damp getting trapped behind which in turn the plaster is absorbing and it is the escaping through this patch on the wall.

    I recommended replacing purely as a cosmetic point of view as in all honesty it would look a lot better than painting it. As rightly said another coat will cover it, just give a light sand then wipe away with a cloth dampened in white spirit.

    Ok re think then lol. Use hardwall as the backing, rule the surface out to an even finish leaving 2-3mm for your skim coat of coarse, then lightly scratch the surface to provide a key for the skim.
