Have a Google of replacement mechanisms, you will see what it looks like inside the window body. ie you need to remove the whole lot to make it flush internally.

No, you adjust all three. There are YouTube vids as to how to do it. Adjustment of each allows for tilt and / or general lift and also lateral movement if required.
