it's sounds like it's not shutting fully when heating not called for. if from cold your hw is calling for heat does the rad side of valve heat up ?

Radiators: feed hot, return cold - what's going on?

sounds like a block on the return. try shutting all but one rad down stairs and remove the trv thermostatic adjuster on that rad. see if that one heats any better. if it does try the next.

Waste pipe from toilet to stack.... horizontal?

in that short distance if it's going straight out the wall in to the soil stack not a problem as the flush will push it that distance . it's when you have a run you can get probs and need a good drop.

Radiators: feed hot, return cold - what's going on?

try the manifold where it's 22mm to 8mm. maybe just air in return side or even slight block that putting all the flow downstairs may move. or add some cleaner to one off the rads and let it circulate for a while.

Installation of shower pump above hot water cylinder

never had one fail yet due to installation problem. and don't use the cheap isolation valves with about 8mm bore get full bore valves. and if you fit any non-return valves never fit before the pump always after the pump. have you got a pump already?

Installation of shower pump above hot water cylinder

as long as your pump is below your cws in loft and you have bleed the pipework at the highest point your only going to get air back in if the cws emptys etc. many pumps are installed in the loft as shower pumps push water not pull
