    Perhaps we shouldn't ask who turned it off? ;) Assuming that the set is plugged into a duplex receptacle (double socket), you might want to try using the other outlet. It's a common arrangement for one half to be switched and the other permanently live.

    What's to stop you from making an observation about something on the report without giving a C1, C2 or C3 code? How do you handle red/black wiring instead of newer brown/blue? Red/black doesn't comply with the current regs., but it hardly warrants a code recommending that it be changed...

    Using countersunk screw heads with a metal back box seems to be all too common, but it's just not the proper type of screw for the application.
