    The DHW settings are fairly limited with the Nest in combi mode. Perhaps I need to have a play around in system boiler mode? I think the settings open up a bit then.

    Nope, and now that you mention it the problem wasn't there when the boiler and heatlink were on but not yet set up. As soon as I got to the equipment stage of setup and confirmed the settings in my last post, the boiler kicked in to heat DHW.
