baxi 105e pressure problem

hi, i've got a baxi 105 e boiler which the pressure keeps rising on and blowing off on press. relief valve anybody got any ideas whats wrong?

If it rises without being on, the secondary heat exchanger is letting mains water get into the CH water.

it is common fault on these boilers Domestic Hot Water heat exchanger
leaking internally.(if the filling loop is o.k)
