concrete slab

When doing a concrete slab, is it best to put a DPM below and another on top prior to screed?
If i just put one DPM below the concrete, how would i run this up when i have the mesh in the way?
I was going for Sub base 150mm,DPM, slab 150mm with b785 . mesh 40mm from sub base then 100mm celotex followed by 75mm screed
