You "should" be able to run your hand over the "nipples" to dislodge the limescale build up.

My favourite limescale remover is HG Blue.

Yes you just pinch any blocked nipples while showering to clear.

A whole house magnetic limescale reducer. Don't know if they work let
us know.
Or water softner

Not all boilers like soft water so be aware of this. Otherwise possibly the best solution

I've fitted Aquabions to a few properties.

Very expensive but easy to fit. Inta do a cheaper version that I'm not convinced about. ( bottom left)

Has anyone got a recommendation for a shower head that can be dismantled for cleaning/descaling etc. All the ones in B&Q seem to be one piece, with rubber spray nozzles that can be rubbed to remove external scale. I want one I can take apart to clean inside, as I live in a very hard water area. Thanks.
