Insulation in extension and main roofs?

What depth of fibre glass insulation is typical for:
1. Flat roofs
2. pitched extn. roofs
3. main pitched roofs.
What U-values would be req by regs for the above?

1. about 500MM (yes half a metre!)0.25 uval
2. 250mm in crossed layers 0.16 uval
3. same as 2.
