I want to install a shower rail above the bath.
The vertical grout line I want to install the screws on is slightly off centred to the right.

1. I have two options, forget the grout line and fit the rail centre ro the bath tab or

2. Install the rail on the grout line and move the adjustable shower head to the left of the rail so the shower head is centred to the bath tab but the rail not.

Option two though has the shower head on the outside towards the bath screen where I usually install it towards the corner wall.

What would you do?

I vote for the screws and rail along the grout line then the shower head will be central. In any case you can angle the shower head so it doesn't play directly on the screen.

Centre, ignore the grout line, get a decent tile drilling bit and use masking tape. Make sure you've got a couple of spare tiles just in case.

Does look good.
I've never understood why people want to only drill holes for fixing stuff in the grout lines of tiles. The holes are always somewhat wider than the grout line, so you've just as much chance (if not more) of cracking the tiles when you try to drill on the grout line. Much better to use some masking tape, use a pointed instrument to indent the glazed surface of the tile, and then drill the hole through the tile using a slow drill speed (and no hammer action either!)
